GuidedTrack does not offer site hosting but it allows you to create programs (think choose-your-own-adventure story or a survey) that can be embedded in your web site.
Products like UpLift, ClearerThinking, and Mind Ease are build using GuidedTrack! You might want to check them out to have a feel for what you can do.
